Develop an online responsive form with multiple design themes, also js encoding required and php var check

I need a web form made of 2-3 pages, like a survey form with 2-3 pages. The form has to be responsive and has to have more than one theme types, like I should change in the page which css to use or something and it will look different , like different styles for input boxes and text. The information inputted by user should be checked in the next page using php. Also fields length should have diff size ( css classes like small xsmall large medium xlarge... ) it would help if a Php function can be created to verify the inputted post information in the field easyer if they have digits or digits and letters or special characters or certain length and so on. The final code should be coded in angular js or something so source it will be encoded. Also I need it all the fields to be able to be changed added or removed and I will change the php code for verification after, because I have some knowledge in all this. More details in PM.
