A form in bootstrap using .Modal

We have a site running bootstrap v 2.2 and we would like to use the .modal option to show a user a form. The form will start with a few fields. Name: [input] Phone : [input] Department: [select list] Depending on the department selected a new set of fields will appear. When the user fills it out they will click submit and a "Thank you message" will appear still with in the .modal view and the submit button will now be a close button and the the user will have the view of the website again.

The dependent fields in the form can be either managed through JS or AJAX within the modal view. I have good experience working with both v 2.x and v 3.x of bootstrap. Need more clarity on the requirements.
