Patients are invited to make a booking via SMS, e-mail or online/App.
Hospital side
1. Login security etc.
2. User based access – admins and standard users
3. Setup clinics: Clinic times (repeating and individual clinics) linked to location and FAQ information. Also setup reminders frequency and linked templates, i.e. you can individualise communications by clinics. Also linked specific surveys and questionnaires.
4. Add patients: manual and integrated from the hospital system options, sent over in status: provisional for booking, or confirmed as booked, including overbookings. Pull info on patient and their referral
5. Booking Engine: automated open bookings to queued patients for booking via SMS, E-mail or online/App. Once the booking period is over, automatically allocate slots to patients, communicate via SMS, e-mail and onle/app and report list of appointments to the hospital, by status (self-book and allocated)
6. Waitlist: automate issuing communication to patients on the waitlist for cancelation re-fulfilment.
7. Communication: standard templates for all communication types to be sent by SMS, email or online/app: on-board, booking open, confirmation, change, cancel, waitlist Can communicate directly to specific patients with read receipt on Email, SMS and in APP
8. Questionnaire: allow for setting up 1 or many questionnaire or surveys for patients with reminders
9. Administration: admins can change a single booking or at full clinic level, with automated or individual communications supported. Admin can make overbookings to clinics: book patients into full clinics – integrated from hospital systems.

7. Waitlist: receive waitlist notifications

10. Add bookings to calendars: Android, iphone, Outlook etc.
11. Reminders: receive notifications for booking, and surveys
12. History: present summary appointment record including status: e.g. attended, no show etc.

1. Do you need a responsive website with a mobile apps? Or just a responsive website should be good?
2. What are the users and user types in the system?
3. You did not mentioned the reporting system needed for an administrator?
4. Is it free or paid?

Basically, you want to develop an online booking system for hospitals, patients will be able to book online appointments and hospitals will be able to register and maintain their profiles.
Here are my questions:
- Which SMS gateway integration is required?
- Do you want to collect online payments while online booking? If yes, then which payment method integration is required? I mean Paypal, Credit Card or any other payment gateway?
- Do you want to collect any payment from hospitals while registration or it will be totally free?
- Can you please explain this task “Add bookings to calendars: Android, iphone, Outlook etc.” in more detail?
- Do you want to develop control for hospital admin to create their own survey questions etc?

You have not mentioned any technical preference there in description. Though every feature requirement was nicely explained for both all user roles viz. patient, hospital staff and admin. All in all this will gonna be complete booking management & appointment scheduling along with records system for this domain (can say hospital management).
I would suggest to pick PHP with one of it's MVC framework like code-igniter, laravel , symphony or any of your choice with angular js front end. Moreover we will provide you proper plan to achieve this goal like starting from designing for approval then back end functionality for modules by taking them step by step.
We prefer to follow AGILE format of work style for such project which allows client to track work progress in a better way.
