HTML/PHP form that updated value in a Mysql Table

Nonconformity database-form We currently have and Excel Sheet that holds 40 columns that is updated by 4 persons. The result is a real mess. nost information are already available so filling the form is redundent operation and most of the time it is not filled rigth. This project is to create a PHP-HTML-AJAX-BOOTSTRAP web form to fill "Nonconformity" data in a "Nonconformity" Mysql Table and also maintain some other tables associated. and also pick the data with ajax call to other database table and sometine insert new record in them. Nonconformity Table: ncid Auto increment int key of the table userid int key of the user table (existing table) Datefilled datetime date of the creation of the document (ncid was provided) TypeEvent int (int form 1 to 8 for event type hard coded in an array) Demandeurid int Key for the Contact table (existing table) clientid decimal (8,2) key for customer table(existing table) blnum int key for packing slip table(existing table) factnum int key for invoice table(existing table) Adress_id int key for a table with the addresse (existing table but need to add record to it sometime directly in the form) nc_date datetime date of the NC (nonconformity) event truckid int key of the truck table(existing table) description text description of the event
