Wordpress - Online form that outputs to Radar chart on a user page

Im in the early stages of setting up a website and im looking for help creating a data visualisation dashboard within word-press that automatically pulls data entered in to a form on a web page by any of my registered customers and then outputs that data on their home page in the form of a radar chart (See image of rough layout and output). So as an example workflow: - User logs into their home page and is presented with a dashboard, here they are able to log a new skill they have learnt via a button which redirects them to another page containing an online form. - The form will contain a number of common fields but there will be a 'category' field which the skill falls under, in this case 'Baking' for example - Under this baking category the user enters a heading into another field for the new skill, example 'Making cookies', they input the process they took for baking cookies in another text field on the form. - They complete the form and submit the log. - They then submit another entry under the 'Baking' category for 'Making a fruit pie' for example, complete the form and submit. - Then on their user homepage they can see a dashboard of how many skills they have learnt for each category. So in this example 'Baking' would show two entries. As more categories are added and more skills added within each category the radar chart displays the areas they are improving in.
