website design (1 page only)

I need one single page to be re-designed. I have a social exchange website with a "surf" (traffic exchange) page that needs a design to match the rest of the website. About the surf page: The user will visit a website, and they will earn coins after the timer finishes. Then they click "Next" to go to the next site. What needs to be done: 1. New design that will match rest of website. 2. section at top for 728x90 banner 3. Need a "report site" link in red. It should look like this at top : Report site | Buy credits | Home (links to be placed somewhere appropriate) 4. New buttons (Skip This Site" and "Next") Please Note: I need minimum disruption to the website as I have members always online, and always joining. It should be quite a basic job for someone with some skill. It is only one single page, with a little design to match the rest of the website.
