Wordpress plugin development using Google Maps

I need to have a Wordpress plugin that lets users search for a Doctor near them, and once they select a one, a PDF with the doctor's data and other info needs to be generated so that the user can print it. The plugin would be something similar to [login to view URL] The plugin will replace the functionality of [login to view URL] (in spanish), where you select your country and city, and then get a list of doctors in your city. When you select a doctor, a PDF is generated and prompts for download. Mi idea is to have both a list and a map (based on Google Maps) were you can see the doctor closest to you. The list would display some of the doctor's information, like address, working hours and the doctor's specialty. You would select a doctor either from the list or the map (a nice call to action that sends data to Google Analytics is needed here). Then, you would go to "Thank you" page, where you would download a PDF with all of the doctor's information (more info than in the list, for example: telephone, e-mail, website). Ideally, I would be able to edit the generated PDF (separate HTML file on the plugin) with some conditional tags (for example, if the doctor is from X city, show Y). I recommend using DOMPDF for the PDF generation. I do all my Wordpress websites based on Bootstrap 3, so using this framework would be ideal. Please read the detailed explanation on the PDF attached.
