We require the construction of an online database. It must be in ASP with sql server.
There will be a secure login section, which takes the user to a switchboard where various functions (such as search data, add data, bulk export/upload) can be performed. There are three main data tables (individual, sample, clinical), with various fields. Please see the attached files for details.
