We need a freelancer to prepare 1 html page with 2 form fields that will turn the plain text inputed by the user into a query string so that a unique link is built and can be sent to our clients. The 2 form fields are: client-name and legal-services. So, the end product will be a single html page that users will be able to input the values to the two form fields in plain text, and then below that area on the page, a custom unique link will display that the user can cut/paste and insert into an email.
