We would require the ftp details as well as the username and admin panel info of wordpress, but if you can provide us with the complete website, we can first test it on out local server and then we can make it live on your server after testing and creating each and everything as you want it to be. But this only will work if you have the original template with documentation :) Our portfolio can be provided on request :)
