Need 2 Dynamic ADOBE ACROBAT X Custom Stamps

We have two Adobe PDF files. We need you to make TWO Adobe Acrobat dynamic custom stamps using these provided PDFs. The stamp should automatically show the date and time of stamping and the user name of the person logged into the computer. The stamp also needs to allow the user to be able to check a box. Please see the attached file to see what the stamp needs to look like and the directions for requirements. Here is a link that shows part of the process needed to make this stamp.
