create survey

I need a survey created for my new cleaning business ( the business is aimed at companies who manage building and engage cleaners and maintenance company for the building, this is an Australian company if you would like to know more about the companies I would like to target please Google strata managers in Sydney), Bidders of this project have to come up with quality questions (I don’t want question like “are you happy with your current cleaner) I want inventive and creative question it will contain 5 questions. I will be offering a gift if they participate in the survey I would like this to be mentioned at the being and the end would be thanking them for their time. Bidders of this project have to have a background in marketing.
