Android Hidden Camera W database

Need an android app that will do the following: User will be prompted to fill out a simple form within android app you create. Once user clicks "begin" to fill out form, secretly turn on front facing camera and start recording video. While camera is secretly in record mode, user of phone will fill out a form. When user clicks "save", all video that was recorded during that "session" will be attached to that simple form. So, the form and video recording will be in one single file saved to the phones memory chip. Along with the video recording, that file will need to have the date and time it was saved AUTOMATICALLY attached. It will get time and date from users phone. The files will need to be sorted by time and date. The form will contain phone numbers and names.
