Build a PHP form with MySQL and store, compute and mail results

I want you to build a PHP and/or Javascript form with: 1) first page with only text and a submit-button to start taking a test (and save the timestamp) 2) second page with 10 questions (10 input text-boxes is fine, you don't need to enter any values) 3) third page with 10 more questions similar to the second page 4) fourth page that asks for 20 input fields, that mimics the functionality for the extra 20 input-fields in: [login to view URL] (and for now it's ok to just reuse the same questions). All in all 40 questions. 5) Then show a 5th "thank you" page, that hiddenly will a) store, b) compute results of the fourth page (look at the page above to see how it's done. They use an "intermediate" page which you don't need to use, you can rather jump to the conclusion directly. The way they've calculated the answers come from the Myers Briggs Personality tests and their values of E<->I, S<->N, T<->F and J<->P. They have done it so that all slide-bars have a name like FeTi, which means that a slider in the left most position will give the F a 100% and the T a 0% and vice versa. The small cap letters e and i will be giving E and I "half its value" c) and e-mail to a pre-determined address together with: i) the results listed from top to bottom from the 2nd and 3rd page ii) the computed result from the fourth page, iii) listed all 20 questions and what answers they had from the fourth page iv) the timestamp of the 5th page minus the first timestamp to show how long time the test took.
