Material estimator tool for heating floor website

Hi, I am looking for help to create a query in a database or any other way to perform this tasks (mysql, php associate table, javascript...) This is a Virtual Assistant (online form) in a site of heating floor. The tool should allow visitors to get a list of product required to complete the our project according to the type of voltage (120/240 volt), the spacing between the heating cable (2.5'' / 3'' / 3.5'' / 4'') and the quantity in square foot to install. First the visitor selects the type of electricity. 120 volt or 240 volt Secondly he choose the space between each heating cable during the installation 2.5'' / 3'' / 3.5 "/ 4'' Thirdly he choose the square feet to cover. After following these choices we must show the list of products they need to buy.
