487180 PHP script Google fix

I have a php script with a google map interface which needs linked to show the address on the google map automatically. It will link the golf course address with the google map already on the webpage ie [login to view URL] This will also be used on several other websites so I need it to be made so I can easily place this code into the script or file when done. This should be a very easy project, if done quickly and perfect I have several other projects to do before Aug 2011.
