Website Page Coding (with database)

I currently have an existing website which basically just inputs database into a table. I need a new page adding to this website (together with a new table for the database). The page has already been designed and you can view it at [login to view URL] This page MUST be in PHP and MySQL. (notice to coders, please do not bid unless you can work in PHP and MySQL). Some notes on the page: 1. Any field that has "date" in the label will need to have a date picker (calendar) and be a date field 2. The field "client name" drop down will be automatically populated from another table in the database. 3. Similarly "venue name" drop down will be automatically populated 4. Similarly with "act/resource name" 5. Similarly the "add resource" button will allow the selection of a name from another table 6. Admin should be able to print this page or email if necessary (if emailing it should be converted into a PDF). A more detailed spec will be provided at a later stage.
