Turn .psd into Wordpress website

Hi, I'd like to have an upgrade for a website. It runs on Wordpress already, but I'd like to have it upgraded to the latest version of Wordpress and to change the design into a new design that I made (see attachment). The template should be an exact copy of the design I attached. Further requirements: - The background should be a repeating background, without unnecessary horizontal scroll bars - The key-visual is a rotating key visual, I'll supply two different photo's (with the handwritten text already on it) - I'd like to add a simple questionnaire with four yes/no questions. Each answer should lead to a specific text OR the respondent goes automatically to the next question. The specific text differs for each question; I'll supply the exact four texts For example: Question 1: Are your shoes yellow?
