LimeSurvey - No Coding Necessary please help with questions - setup

Hi, I need to work with a coder who is familiar with LimeSurvey. We have installed the software and setup a Survey. Now we need some help formatting the questions. This is a job application survey. We have the following question: Please list previous addresses that you have had in the last 5 years: [ADDRESS ] [ CITY ] [ STATE ] [ ZIP ] Address - 1 line text City - 1 line text State - Dropdown Zip - 1 line text Ideally we would like to list all these fields on one line, and have 5 lines with address, city, state, zip fields repeating. Could you help me set this up? Also, we would like to setup this survey so that anybody on the internet can come and take it. THANKS!
