We wish to develop a small application, which can help our website visitors do a self-evaluation of their present finance function. We have created a questionnaire as well as certain data points, and based on the responses, a rating will be calculated and feedback given. A report will be generated, populating these responses and the analysis of the same and mailed across to these visitors (taking the test) in an automated fashion. The analysis of the responses will be done by the application and the interpretation of each response can be either stand-alone (i.e. single interpretation) or multiple (i.e. dependent on other responses or selections by the visitors for other questions elsewhere). While we have defined these factors for all, this intelligence will need to be built into the application, for it to evaluate and prepare the report. Requirement: 1. Deployment a. Should be part of MyCFO corporate Website. 2. Usages a. Website visitor 3. Input Sheet (part I), a. to collect the mandatory information of visitor 4. Input Sheet (part II), a. to collect additional information of the visitor’s company. These questions are optional. 5. Questionnaire, a. Multiple choice questions segregated into multiple categories. 6. Admin ID would be required to access visitor information and make changes to the questionnaire/ interpretation of responses if required 7. Output A report will be generated basis the responses given. Certain graphics can also be part of the report. The report will need to be generated (populated in pre-defined fields) automatically and mailed across to the email ID shared earlier by the visitor with a blind copy to the Admin.
