I have an excel spreadsheet with 7 columns, one column has a pass/fail checkbox and two columns require signatures. Currently this spreadsheet is printed and filled in by hand where the information in the first three columns are checked, will receive a pass or a fail result and two people put in their initials and date to confirm the result. There is also a cover page with approval signatures. I want to be able to convert this spreadsheet to a form that can be filled in using a tablet, including electronic signatures. The information in the spreadsheet is going to be changing constantly so a new form will need to be created every week or so.
